Informed Consent

Physical therapists have completed a Bachelor, Masters or Doctorate degree program of study and passed a state licensure examination. We are not physicians and do not order radiological or laboratory tests, or prescribe medication although we work closely with referring physicians and can suggest such treatments if necessary.
Physical therapists are trained to evaluate, test and measure the function of the musculoskeletal, neurological and cardiovascular systems. A variety of hands-on techniques, therapeutic exercises, and /or machines are utilized to correct posture, control pain and restore movement and function. Physical therapy includes services rendered by support staff under the supervision and direction of the physical therapist.


Late Policy

Being late by more than 10 minutes will require you to either reschedule or wait for the next available opening. We do not allow appointments to overlap because this undeservedly compromises the individual and personalized care of another patient.


24 Hour Advance Notice Fee

If you wish to change or cancel an appointment we require a minimum of 24 hour notice. Missed appointments without a phone call or notification will result in a $50.00 no call no show fee which is due before you are seen for your next scheduled visit. If we do not receive to proper notice we will charge a same day cancellation fee of $20.
